It's a reference to the confusion that has arisen in the West's sexual politics (and more particularly the sexual politics of the USA, given the image) as a result of the deliberate conflation of biological sex (a genetically determined reality) with gender (a social construct shaped by social norms and stereotypes of masculinity and femininity) that has accompanied the rise of trans(sexual) ideology (among other manifestations) and its absurd claim that one's sex is whatever one "identifies" as, regardless of one's chromosomal makeup.

The "trans rights" movement (which is predominantly made up of men "identifying" as women) has accordingly asserted that spaces, institutions and institutional arrangements which have been specifically set up for the benefit of actual females should also be accessible to men pretending to be women. So this includes such things as the "right" for them to enter female toilets, changing/locker rooms, female sports and sports leagues, domestic violence shelters and women's prisons, as well as access to positive discrimination measures intended to benefit women and girls, inclusion in official statistics about gender-based (or rather, biological sex-based violence), and entry into organizations run by and for females like the UK's Women's Institute. (It's noteworthy that the corresponding claims made by women pretending to be men are just a faint echo of the clamour raised by men pretending to be women.)

In the photo above, the caption implies that the speaker is not confused about which bathroom he should use because he is confident in his masculinity and his maleness. (Apparently, he also thinks a real man also fetishizes the use and ownership of firearms.)

(Sorry for the lengthy explanation, but I felt that the answer to your question required giving a lot of contextual background info in order to clarify what the caption implies.)

In my opinion, all this is connected with the moral and cultural relativism that was mainstreamed as every aspect of life began to be treated as a consumer choice: not just in relation to physical goods, but to personal identity and lifestyles, as the traditional markets and investment opportunities available to modern industrial capitalism matured or shrank to the point where there was not much profit left to be squeezed out of them. This has led companies to look for ways to commodify and monetize individuals, their values and their lifestyles (an integral aspect of what is termed neoliberalism or neoliberal economics, which recognizes only the primacy of making money and disregards the human, societal and environmental costs of pursuing that aim).

Hence, for instance, the mainstreaming of pornography and the growing acceptance of prostitution as just another lifestyle choice (despite the gross exploitation, misogyny and power imbalances involved in both those industries), and the deliberate cultivation and encouragement of gender confusion among children and teenagers.

This confusion is designed to generate -- particularly in the USA and to some extent Canada, where it is being pushed by lobbying from pharmaceutical companies, organizations representing hospitals and medical personnel, and the aforementioned "trans rights" activists -- a profitable lifetime pipeline of hormone treatments, sex reassignment surgery and stabilizing medications.

All this regardless of the pain, mental and emotional anguish, social rejection and/or isolation, physical disfigurement, infertility and likelihood of medical complications that is inflicted on the individuals victimized by this ruthless money-motivated gold rush, which treats humans with the same callous disregard with which we usually treat animals and the environment.
