a. I made mistakes three times.
b. Three times I made mistakes.

c. I made mistakes on three occasions.
d. On three occasions I made mistakes.


Could one use these sentences if one had made three mistakes in all?
Could one use these sentences if one had made -say- five mistakes in all? (at least on one occasion there was only one mistake)

One could use these sentences, though he'd have to say four, then.

I think most native speakers would use "a mistake" in all cases if a single mistake on three occasions is meant.
